Soon, PHS will be 50 years old! Our golden anniversary will be more meaningful if we can celebrate it with the entire PHS family: the students, the alumni, the parents and our former and incumbent teachers and administrators. To be able to do this, we need to know where you are now and how we can reach you.

This Form also serves as your application for the ALUMNI CARD. Please make sure that you fill up the bottom part and send also a 1x1 colored picture with white background when you submit this form. The Alumni Card costs P50.00 only (not yet available).
